How it works

Research suggests that thymosin alpha-1 can influence both innate and adaptive immune responses. Earlier pharmacological studies described effects on T-cell differentiation and function, and subsequent research has investigated its effects on other immune cells and signaling pathways involved in the body's response to infection and inflammation. These findings provide a basis for studying thymosin alpha-1 as an immunomodulatory peptide; they do not establish that the peptide can prevent, treat, or cure a specific disease.

About Thymosin A-1

Thymosin A-1 25 mg is a formulation containing thymosin alpha-1 (Tα1), a synthetic 28-amino-acid peptide studied for its immunomodulatory effects. Research has examined thymosin alpha-1 in infectious diseases, sepsis, cancer, and other conditions involving altered immune responses. Thymosin A-1 is not FDA-approved in the United States. A licensed healthcare provider can determine whether treatment is appropriate based on an individual's medical history and applicable prescribing and compounding requirements.

Note: The 25 mg designation refers to the formulation strength. It does not represent a recommended patient dose.

What Is Thymosin A-1?

Thymosin alpha-1, also called thymalfasin or Tα1, is a synthetic peptide containing 28 amino acids. Researchers have studied it for its ability to influence different components of the immune system.

Unlike a conventional immune stimulant, thymosin alpha-1 is generally described as an immunomodulatory peptide. Research has examined its effects on T-cell activity and other immune pathways involved in host defense.

The clinical effects of thymosin alpha-1 may vary based on the underlying condition, patient population, and treatment setting. Therefore, research findings should not be generalized to all individuals or conditions.

How Does Thymosin A-1 Work?

Research suggests that thymosin alpha-1 can influence both innate and adaptive immune responses.

Earlier pharmacological studies described effects on T-cell differentiation and function. Subsequent research has investigated its effects on other immune cells and signaling pathways involved in the body's response to infection and inflammation.

These findings provide a basis for studying thymosin alpha-1 as an immunomodulatory peptide. They do not establish that the peptide can prevent, treat, or cure a specific disease.

What Does the Research Say?

Thymosin alpha-1 has been studied in human clinical trials for several decades. A 2024 review examined more than 30 clinical trials involving more than 11,000 participants and reviewed its safety and potential clinical applications across several disease areas.

Research has included infectious diseases, sepsis, cancer, and other conditions involving immune dysfunction. However, results have differed between clinical settings, and evidence for one condition should not automatically be applied to another.

Thymosin A-1 and Sepsis Research

Sepsis has been one of the areas receiving substantial research attention.

A 2025 systematic review and meta-analysis included 11 randomized controlled trials involving 1,927 patients. The overall analysis found an association between thymosin alpha-1 and lower 28-day mortality. However, the mortality difference was not statistically significant in higher-quality and multicenter subgroup analyses, and the authors concluded that additional research is needed.

Importantly, a large 2025 phase 3 randomized, double-blind, placebo-controlled trial included 1,089 adults with sepsis. The study found no clear evidence that thymosin alpha-1 reduced 28-day all-cause mortality compared with placebo.

This more recent evidence is important because it demonstrates why claims about thymosin alpha-1 and sepsis should remain cautious.

Thymosin A-1 and Immune Function

Researchers have investigated thymosin alpha-1 because of its potential effects on immune regulation.

Studies have examined changes in T-cell function and other immune responses, particularly in people experiencing immune dysfunction. However, describing thymosin A-1 simply as an "immune booster" can oversimplify the scientific evidence. The more accurate description is immune modulation.

The available research does not establish that thymosin A-1 improves immune function in healthy people or prevents infections.

FDA Status of Thymosin A-1

Thymosin alpha-1 is not FDA-approved in the United States.

The FDA Orphan Drug Designation database contains historical orphan designations for thymalfasin, including designations related to chronic active hepatitis B, DiGeorge anomaly with immune defects, hepatocellular carcinoma, and malignant melanoma. The FDA records identify these as orphan designations and state that they were not FDA-approved for those orphan indications.

An orphan-drug designation should therefore not be described as FDA approval.

FDA documents also show that thymosin alpha-1 has been considered in the context of pharmacy compounding. However, the regulatory status of a specific compounded formulation depends on the applicable federal requirements and the circumstances of compounding. This page should not imply that FDA consideration of an ingredient establishes approval of a particular compounded product.

Safety and Side Effects

Clinical research has generally described thymosin alpha-1 as well tolerated, although safety findings vary among studies and patient populations. Earlier clinical literature reported local irritation at the injection site among commonly observed adverse effects.

Because thymosin alpha-1 can influence immune activity, its use may require additional consideration in people with immune-related conditions or those receiving therapies that affect immune function.

Patients should discuss their medical history, current medications, allergies, and potential risks with a qualified healthcare provider before treatment.

Is Thymosin A-1 25 mg FDA Approved?

No. Thymosin A-1 25 mg is not an FDA-approved drug product.

Historical FDA orphan-drug designations for thymalfasin do not constitute FDA approval.

The 25 mg designation describes the formulation strength and should not be presented as an FDA-approved dosage or recommended patient dose.

Is Thymosin A-1 the Same as Thymosin Alpha-1?

Yes. Thymosin A-1, thymosin alpha-1, Tα1, and thymalfasin are names used for the same peptide in the scientific and medical literature.
