Key potential benefits

How it works

Tesamorelin mimics the activity of endogenous growth hormone-releasing hormone. It stimulates the pituitary gland to release growth hormone, which can subsequently increase levels of insulin-like growth factor 1 (IGF-1). Research has examined this pathway because of its relationship with body composition and visceral adipose tissue; clinical evidence is strongest in adults with HIV-associated lipodystrophy.

About Tesamorelin

Tesamorelin 15 mg is a compounded formulation containing tesamorelin, a synthetic growth hormone-releasing factor (GHRF) analog. Tesamorelin stimulates the release of growth hormone and has been studied for its effects on visceral adipose tissue. The FDA-approved tesamorelin product, EGRIFTA WR, is indicated to reduce excess abdominal fat in adults with HIV-associated lipodystrophy. It is not indicated for general weight-loss management.

Tesamorelin 15 mg should be considered only after evaluation by a licensed healthcare provider. The FDA approval of EGRIFTA WR applies to that specific product, formulation, and approved indication. It does not mean that a separately compounded tesamorelin 15 mg formulation is FDA-approved.

What Is Tesamorelin 15 mg?

Tesamorelin is a synthetic analog of growth hormone-releasing hormone. It acts on receptors involved in the release of growth hormone from the pituitary gland.

The 15 mg designation refers to the amount of tesamorelin contained in the product, rather than establishing an individual patient's treatment dose. The appropriate administration and dosing schedule should be determined by the prescribing clinician based on the specific formulation and patient's circumstances.

How Does Tesamorelin Work?

Tesamorelin mimics the activity of endogenous growth hormone-releasing hormone. It stimulates the pituitary gland to release growth hormone, which can subsequently increase levels of insulin-like growth factor 1 (IGF-1).

Research has examined this pathway because of its relationship with body composition and visceral adipose tissue. Clinical evidence is strongest in adults with HIV-associated lipodystrophy.

What Does the Research Say?

Clinical trials provide evidence for tesamorelin's effect on visceral adipose tissue in specific patient populations.

A randomized, placebo-controlled trial published in The New England Journal of Medicine included 412 adults with HIV and excess abdominal fat. After 26 weeks, visceral adipose tissue decreased by 15.2% with tesamorelin compared with a 5.0% increase with placebo.

Another randomized clinical trial involving 50 adults with HIV-associated abdominal fat accumulation found that six months of tesamorelin significantly reduced visceral adipose tissue and liver fat compared with placebo.

A 2026 systematic review and meta-analysis of randomized controlled trials also found reductions in visceral adipose tissue, waist circumference, and trunk fat among people with HIV-associated lipodystrophy. These findings should not be generalized to people without this condition.

Is Tesamorelin FDA-Approved?

Yes, tesamorelin is the active ingredient in an FDA-approved drug product.

The FDA-approved product EGRIFTA WR is indicated for reducing excess abdominal fat in HIV-infected adults with lipodystrophy. The prescribing information specifically states that it is not indicated for weight-loss management.

However, this distinction is important for a Tesamorelin 15 mg compounded product:

FDA approval of EGRIFTA WR does not mean that a compounded Tesamorelin 15 mg formulation is FDA-approved.

The formulation, concentration, route, manufacturing process, and intended use may differ from the approved product.

Tesamorelin 15 mg and Visceral Fat

Visceral adipose tissue is fat stored around internal abdominal organs. It differs from subcutaneous fat, which is located beneath the skin.

Tesamorelin has been studied for its effects on visceral adipose tissue, particularly in people with HIV-associated lipodystrophy.

This evidence should not be converted into claims that Tesamorelin 15 mg will produce a specific amount of abdominal-fat reduction in an individual patient.

Safety and Side Effects

Tesamorelin affects the growth hormone and IGF-1 pathways, so medical supervision is important.

Reported and clinically relevant risks include:

  • Injection-site reactions
  • Joint pain
  • Muscle pain
  • Peripheral edema
  • Carpal tunnel syndrome
  • Increased IGF-1 levels
  • Glucose intolerance
  • Diabetes
  • Hypersensitivity reactions

The FDA prescribing information also notes that the long-term cardiovascular safety of EGRIFTA WR has not been established.

Patients should discuss relevant medical conditions and medications with their healthcare provider before treatment.

Tesamorelin 15 mg and Blood Sugar

Tesamorelin can affect glucose metabolism. The FDA prescribing information includes warnings concerning glucose intolerance and diabetes.

A healthcare provider can determine whether glucose monitoring or other clinical evaluation is appropriate based on the patient's individual circumstances.

Why Clinical Evaluation Matters

Tesamorelin treatment should be considered only after a licensed healthcare provider evaluates the patient's medical history, current medications, relevant medical conditions, and treatment needs.

If a compounded Tesamorelin 15 mg formulation is being considered, the clinician should also determine whether compounding is appropriate under applicable requirements.

Optimal Club's role should be presented as providing access to clinician evaluation, rather than guaranteeing that Tesamorelin 15 mg is appropriate for a particular patient.
